A man charged with attempted murder escaped from hospital custody last month, prompting a federal review and a pledge from the hospital to change its procedures. The Oregon Health Authority released a corrective plan, including the results of a federal investigation which found the man escaped in part because staff left the keys in the ignition of the van he was being transported in.
