Lt. Merrill Thompson said troopers were called on a report of a vehicle in a pond near a warehouse on Park West Drive, just off U.S. Route 40, around 6:25 p.m.
Witnesses told investigators they saw a vehicle, which OSHP said was a 2016 Jeep Cherokee, traveling westbound in the left lane before slowly driving to the right lane, off the road and into the pond. Investigators added that the vehicle struck a ditch before coming to a stop in the pond.
Three adults and two children were inside the Cherokee at the time of the crash, according to OSHP.
Thompson said the driver was reportedly able to get out of the vehicle, and a trooper was able to rescue one of the children before the vehicle sank. He added that another person was rescued after the car was fully submerged. An adult and a 3-year-old child were eventually pulled from the vehicle, but it’s unclear when it happened.
The adult, later identified as 27-year-old Kelita Alfred, and the 3-year-old child were taken to Doctors West where they were pronounced dead.
The other adults were not injured in the crash, according to OSHP. The other child, a 5-year-old, was taken to Nationwide Children’s Hospital with injuries that were not life-threatening.
OSHP West Jefferson Post, West Jefferson Police Department, Madison County Sheriff's Office, Jefferson Township Fire and EMS and Prairie Township Fire Department assisted in the water rescue.
The incident remains under investigation.
